ABSTRACT

Fundamento: la tirosinemia hereditaria tipo I o tirosinemia hepato-renal es una enfermedad autosómica recesiva causada por la deficiencia de la enzima fumarilacetoacetato hidrolasa. Debido a su complejidad metabólica, su confirmación lleva aparejado, un conjunto de métodos altamente costosos. Objetivo: implementar una metodología de trabajo para la detección de metabolitos marcadores de la tirosinemia tipo 1. Método: se realizó un estudio descriptivo y transversal en una serie de casos evaluados en el período comprendido entre enero del 2021 a febrero del 2023. Como examen inicial se realizaron las pruebas cualitativas de α nitroso beta naftol y 2,4 dinitrofenilhidracina para la identificación de la tirosina y los α cetoácidos respectivamente. Luego se realizó el método HPLC para la cuantificación de la tirosina y CG-EM para la determinación del perfil cromatográfico en orina como técnicas confirmatorias. Resultados las pruebas cualitativas resultaron positivas para la tirosina y sus metabolitos, así como, para los alfa cetoácidos. El perfil de ácidos orgánicos mostró excreción elevada de los metabolitos marcadores de la enfermedad en 8 casos, a los cuales se les fue realizada la cuantificación de la tirosina que resultaron positivos de padecer la enfermedad. Conclusiones: la implementación de la metodología resultó ser una herramienta valiosa en el diagnóstico temprano de la enfermedad.


Foundation: hereditary tyrosinemia type I or hepato-renal tyrosinemia is an autosomal recessive disease caused by deficiency of the enzyme fumarylacetoacetate hydrolase. Due to its metabolic complexity, its confirmation requires a set of highly expensive methods. Objective: to implement a work methodology for the detection of marker metabolites of type 1 tyrosinemia. Method: a descriptive and cross-sectional study was carried out in a series of cases evaluated in the period from January 2021 to February 2023. As an initial examination, qualitative tests of α nitroso beta naphthol and 2,4 dinitrophenylhydrazine were carried out for the identification of tyrosine and α keto acids respectively. Then, the HPLC method was performed for the quantification of tyrosine and GC-MS for the determination of the chromatographic profile in urine as confirmatory techniques. Results: qualitative tests were positive for tyrosine and its metabolites, as well as for alpha keto acids. The organic acid profile showed elevated excretion of the disease marker metabolites in 8 cases, in which tyrosine quantification was performed, which were positive for suffering from the disease. Conclusions: the implementation of the methodology turned out to be a valuable tool in the early diagnosis of the disease.

La sarcopenia asociada a la edad es una condición clínica caracterizada por una disminución en la fuerza, calidad y cantidad de masa muscular así como también en la función muscular. Un biomarcador se define como una característica que es medible objetivamente y evaluable como indicador de un proceso biológico normal, patológico o respuesta terapéutica a una intervención farmacológica. Los marcadores bioquímicos propuestos para el estudio de la sarcopenia pueden ser categorizados en dos grupos. El primero de ellos evalúa el estatus musculoesquelético; este panel de marcadores está formado por miostatina/folistatina, procolágeno aminoterminal tipo III e índice de sarcopenia. El segundo grupo de marcadores bioquímicos evalúa factores causales, para lo cual se sugiere medir el factor de crecimiento insulino-símil tipo 1 (IGF-1), dehidroepiandrosterona (DHEAS), cortisol, facto-res inflamatorios [proteína C reactiva (PCR), interleuquina 6 (IL-6) y factor de necrosis tu-moral (TNF-a)]. Las recomendaciones realiza-das están basadas en la evidencia científica disponible en la actualidad y la disponibilidad de la metodología apropiada para cada uno de los biomarcadores. (AU)


Sarcopenia is a progressive and generalized skeletal muscle disorder defined by decrease in the strength, quality and quantity of muscle mass as well as in muscle function. A biomarker is defined as a feature objectively measured and evaluated as an indicator of a normal biologic process, a pathogenic process or a pharmacologic response to therapeutic intervention. The biochemical markers proposed for the study of sarcopenia may be classified in two groups. The first group evaluates the musculoskeletal status, made up by myostatin/follistatin, N-terminal Type III Procollagen and the sarcopenia index. The second evaluates causal factors, where the measurement of the following is suggested: hormones insulin-like growth factor-1 (IGF-I), dehydroepiandrosterone sulphate (DHEAS), cortisol, inflammatory factors [C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-a (TNF-a)]. The recommendations made are based on scientific evidence currently available and the appropriate methodology availability for each biomarker. (AU)

ABSTRACT

Objective:To study the correlation of bone metabolism biomarkers between newborn twins and their mothers during pregnancy.Methods:From January 1, 2018 to June 30, 2022, newborn twins with mild symptoms admitted to the neonatal department of our hospital were retrospectively reviewed. The clinical data of the twins and their mothers were collected, including bone metabolism biomarkers of the twins within 3 d after birth and their mothers within last month during pregnancy. The twins were assigned into different groups according to gestational age(GA), birth weight(BW), the relationship between BW and GA(appropriate for GA(AGA),small for GA(SGA) and large for GA(LGA), birth season, gender, and the mothers' age, ethnicity, pre-delivery body mass index (BMI), gestational BMI increase, number of births and chorionic properties. The correlations of bone metabolism biomarkers between the twins and their mothers were analyzed.Results:A total of 302 pairs of twins were included. The incidence of insufficient or deficient serum 25-(OH)D 3 was 97.4% among the mothers, and 87.7% among the twins. The levels of blood phosphorus ( r=0.262, P<0.001) and 25-(OH)D 3 ( r=0.239, P=0.002) in mothers were positively correlated with the twin with larger BW. No significant differences existed in 25-(OH)D 3 between genders, AGA,SGA and LGA, birth season, and mothers' age, ethnicity, pre-delivery BMI, gestational BMI increase and chorionic properties( P>0.05). 25-(OH)D 3 in the twins were positively correlated with BW and 25-(OH)D 3 of mothers before delivery ( P<0.05) and negatively correlated with number of births ( P<0.05). Conclusions:In most mothers and their newborn twins, 25-(OH)D 3 are insufficient or deficient. The levels of blood phosphorus and 25-(OH) D 3 are correlated between the newborns and their mothers. The lower the BW of the newborn, the more times the mother give birth and the lower the mother's pre-delivery 25-(OH)D 3 level, the lower the newborn's 25-(OH)D 3 level.

ABSTRACT Introduction: Taekwondo is a fighting and confrontational game that requires exceptionally high physical and psychological qualities from athletes. It has higher requirements on the athlete's explosive power, coordination ability, spatial perception, and psychosocial quality. Any slight body changes in an athlete can destabilize the nervous system and cause changes in the immune system. These changes can increase the risk factor during training and competition. Objective: Monitor the physical function of taekwondo athletes. Methods: This paper discusses the essential role of biochemical indicators in taekwondo training. At the same time, it explores the relationship between physical function and the training effect of taekwondo athletes. Taekwondo athletes underwent a one-year training program. The program monitored and recorded the athletes' biochemical parameters during training. The relationship between the training program formulation and the biochemical indicators of taekwondo athletes was analyzed. Results: The overall average level of nitrogen and urea in the blood was significantly higher than that of the setting phase after a large amount of exercise (P<0.05). Changes in blood urea after the routine were relatively stable (P>0.05). Most participants demonstrated hemoglobin in the normal range and reached its upper limit after winter training. Conclusion: The quality of the biochemical indicators of the athlete directly affects his performance. Sanguine nitrogen and hemoglobin are the main biochemical indicators for monitoring Taekwondo athletes. Level of evidence II; Therapeutic studies - investigation of treatment outcomes.


RESUMO Introdução: O Taekwondo é um jogo de luta e confronto que requer qualidades físicas e psicológicas excepcionalmente elevadas dos atletas. Ele tem requisitos mais altos sobre o poder explosivo do atleta, capacidade de coordenação, percepção espacial e qualidade psicossocial. Qualquer leve alteração corporal de um atleta pode desestabilizar o sistema nervoso e causar alterações no sistema imunológico. Estas mudanças podem aumentar o fator de risco durante o treinamento e a competição. Objetivo: Monitorar a função física dos atletas de taekwondo. Métodos: Este documento discute o papel essencial dos indicadores bioquímicos no treinamento de taekwondo. Ao mesmo tempo, explora a relação entre a função física e o efeito do treinamento dos atletas de taekwondo. Atletas de taekwondo foram submetidos a um programa de treinamento de um ano. O programa monitorou e registrou os parâmetros bioquímicos dos atletas durante o treinamento. Analisou-se a relação entre a formulação dos programas de treinamento e os indicadores bioquímicos dos atletas de taekwondo. Resultados: O nível médio geral de nitrogênio e ureia no sangue foi significativamente maior do que o da fase de ajuste após uma grande quantidade de exercício (P<0,05). As variações na ureia sanguínea após a rotina foram relativamente estáveis (P>0,05). A maioria dos participantes demonstrou hemoglobina na faixa normal e atingiu o seu limite superior após o treinamento de inverno. Conclusão: A qualidade dos indicadores bioquímicos do atleta afeta diretamente o seu desempenho. O nitrogênio e a hemoglobina sanguíneos podem ser usados como os principais indicadores bioquímicos para monitorar os atletas de Taekwondo. RESUMEN Introducción: El taekwondo es un juego de lucha y enfrentamiento que exige a los deportistas unas cualidades físicas y psicológicas excepcionalmente altas. Tiene mayores exigencias en cuanto a la potencia explosiva, la capacidad de coordinación, la percepción espacial y la calidad psicosocial del atleta. Cualquier pequeño cambio corporal en un deportista puede desestabilizar el sistema nervioso y provocar cambios en el sistema inmunitario. Estos cambios pueden aumentar el factor de riesgo durante el entrenamiento y la competición. Objetivo: controlar la función física de los atletas de taekwondo. Métodos: Este artículo analiza el papel esencial de los indicadores bioquímicos en el entrenamiento de taekwondo. Al mismo tiempo, explora la relación entre la función física y el efecto del entrenamiento de los atletas de taekwondo. Los atletas de taekwondo se sometieron a un programa de entrenamiento de un año. El programa controló y registró los parámetros bioquímicos de los atletas durante el entrenamiento. Se analizó la relación entre la formulación de los programas de entrenamiento y los indicadores bioquímicos de los atletas de taekwondo. Resultados: El nivel medio global de nitrógeno y urea en sangre fue significativamente mayor que el de la fase de ajuste después de una gran cantidad de ejercicio (P<0,05). Los cambios en la urea en sangre después de la rutina fueron relativamente estables (P>0,05). La mayoría de los participantes mostraron una hemoglobina en el rango normal y alcanzaron su límite superior después del entrenamiento invernal. Conclusión: La calidad de los indicadores bioquímicos del deportista afecta directamente a su rendimiento. El nitrógeno sanguíneo y la hemoglobina pueden utilizarse como principales indicadores bioquímicos para el seguimiento de los atletas de taekwondo. Abstract Objectives Evidence suggests that ketamine's influence on brain-derived neurotrophic factor (BDNF) might be involved in its mechanism of rapid antidepressant action. We aimed to evaluate the differential impact of ketamine and esketamine on serum BDNF levels and its association with response patterns in treatment-resistant depression (TRD). Methods Participants (n = 53) are from a randomized, double-blind clinical trial comparing the efficacy of single-dose ketamine (0.5mg/kg, n = 27) and esketamine (0.25mg/kg, n = 26) in TRD. Depression severity was assessed before and 24 hours, 72 hours, and 7 days after the intervention, using the Montgomery-Åsberg Depression Rating Scale (MADRS). Blood samples were collected before infusion, 24 hours, and 7 days afterwards. Results There were no significant changes in BDNF levels at post-infusion evaluation points, and no difference in BDNF levels comparing ketamine and esketamine. Both drugs exhibited similar therapeutic effect. There was no association between BDNF levels and response to treatment or severity of depressive symptoms. Conclusion There was no significant treatment impact on BDNF serum levels - neither with ketamine nor esketamine - despite therapeutic response. These results suggest that ketamine or esketamine intervention for TRD has no impact on BDNF levels measured at 24 hours and 7 days after the infusion. @#Aim and design: L-carnitine supplementation combined with exercise may enhance metabolic responses and improve exercise performance. Thus, this systematic review article aims to identify the effects of the L-carnitine supplement on biochemical markers, and exercise performance when combined with exercise. Data Sources: The articles are screened and reviewed based on titles, abstracts and keywords. Only peer-reviewed studies written in the English language, dated January 2000 to March 2021, were considered in this review. Review Methods: The PRISMA method is used for this study. PubMed, EBSCOHost, SpringerLINK, and Scopus databases were used to systematically search. Results: From a search 731 articles, 12 articles were identified related to effect of L-carnitine intake with exercises on biochemical markers and exercise performance. Studies showed that L-carnitine supplementation can be consumed daily during aerobic or anaerobic exercises by different population including patients. Its combination could improve serum lipid profiles, antioxidant properties, markers of metabolic, oxidative stress, and inflammatory, and exercise performance. Conclusion and Impact: This review provides information regarding the beneficial effects of L-carnitine supplement with recommended dosages and exercise prescriptions on overall body health in human.

RESUMEN Fundamento: la resistencia a la insulina y las alteraciones metabólicas asociadas, como la dislipemia, la hipertensión, la obesidad y la hipercoagulabilidad, influyen en la aparición cada vez más prematura y la severidad de la aterosclerosis que desarrollan los pacientes con diabetes mellitus Objetivo: evaluar los marcadores bioquímicos de aterosclerosis subclínica en pacientes con diabetes mellitus tipo 2. Método: se realizó un estudio descriptivo y transversal para evaluar marcadores bioquímicos de aterosclerosis subclínica en 100 pacientes con diabetes mellitus tipo 2 atendidos en el Hospital General Docente Guillermo Domínguez López desde abril del 2020 hasta marzo del 2021. Las variables analizadas fueron: edad, sexo, tensión arterial, antecedentes patológicos personales de diabetes mellitus, hábitos tóxicos y riesgo cardiovascular. Estos pacientes fueron clasificados según su riesgo cardiovascular global utilizando las gráficas de riesgo que establece la Organización Mundial de la Salud para la región de las Américas y se les realizaron las determinaciones de colesterol total, triglicéridos, LDL colesterol, HDL colesterol, creatinina, ácido úrico, cistatina C, fibrinógeno, proteína C reactiva de alta sensibilidad. La información se procesó mediante métodos de estadística descriptiva, principalmente tasas y porcientos. Resultados: la mayoría de los pacientes se clasificaron en riesgo cardiovascular global 2 y 3. Sus marcadores mostraron asociación con las categorías de riesgo establecidas en cada paciente. Las determinaciones de triglicéridos, HDL-colesterol y ácido úrico fueron las más significativas para identificar el daño vascular en los pacientes con diabetes mellitus tipo 2. Conclusiones: hubo una relación directa entre los valores alterados de la proteína C reactiva de alta sensibilidad y el riesgo cardiovascular global en los pacientes estudiados.


ABSTRACT Background: insulin resistance and associated metabolic alterations, such as dyslipidemia, hypertension, obesity and hypercoagulability, influence the increasingly premature appearance and severity of atherosclerosis that patients with diabetes mellitus develop. Objective: to evaluate the biochemical markers of subclinical atherosclerosis in patients with type 2 diabetes mellitus. Method: a descriptive and cross-sectional study was carried out to evaluate biochemical markers of subclinical atherosclerosis in 100 patients with type 2 diabetes mellitus treated at the Guillermo Dominguez López General Teaching Hospital from April 2020 to March 2021. The used variables were: age, sex, blood pressure, personal medical history of diabetes mellitus, toxic habits and cardiovascular risk. These patients were classified according to their global cardiovascular risk using the risk charts established by the World Health Organization for the Americas region and total cholesterol, triglycerides, LDL cholesterol, HDL cholesterol, creatinine, uric acid determinations were performed, cystatin C, fibrinogen, high-sensitivity C-reactive protein. The information was processed using descriptive statistics methods, mainly rates and percentages. Results: most of the patients were classified in global cardiovascular risk 2 and 3. Their markers showed association with the risk categories established in each patient. The determinations of triglycerides, HDL-cholesterol and uric acid were the most significant to identify vascular damage in patients with type 2 diabetes mellitus. Conclusions: there was a direct relationship between altered values ​​of high-sensitivity C-reactive protein and global cardiovascular risk in the studied patients.

Background: Pandemics are natural calamities, which humans have been encountering since time im-memorial. The world has witnessed and endured several pandemics like from Justinian Plague to Spanish Flu. The 21st century has already witnessed three major pandemics like Severe Acute Respiratory Syndrome in 2002, Middle East Respiratory syndrome in 2012 and at present COVID?19. The first case of COVID?19 was reported in Wuhan, Hubein Province in Dec 2019. The first case of COVID?19 was reported in Kerala on 30th January 2020. COVID?19 virus is a single stranded RNA Virus and resembles SARS -2 virus and hence the name SARS -COV 2. There has been a report of around 34 million cases with 1 million deaths. Majority of the deaths occur in the elderly population as they have immunosenescence and multiple comorbidities. Hence a study aiming at analysis of correlation between SpO2, radiological and biochemical markers is essential in management of COVID 19 in elderly. Aims & Objectives: To study the correlation between SpO2 and radiological features in X ray and CT Chest and biochemical markers with the severity and outcome in geriatric patients. Study Period: April 2020 to September 2020, Duration: 6 months. Study Centre: Chengalpattu Medical College, Tamilnadu. Inclusion Criteria: RT PCR positive Covid 19 patients more than 60 years of age. Exclusion Criteria: Covid 19 patients less than 60 years, RT PCR Negative patients. Materials and Methods: Retrospective analysis of records of 300 Covid 19 patients, aged more than 60 years hospitalized in Chengalpattu Medical College during April to September 2020 was done and biochemical parameters mentioned below were corelated with features in CXR and CT Chest RTPCR positive spo2 and analysis with severity of disease and outcome was done. Results and Discussion: On retrospective analysis of records of elderly Covid 19 patients, it was observed that the elderly could be graded as mild, moderate and severe based on clinical criteria. Radiological parameters co-relating in the same group classified as mild, moderate and severe could be derived and could be used for assessment of patients. Similarly, it was observed that the biochemical parameters of D? Dimer, C reactive protein, Neutrophil Lymphocyte Ratio correlated most significantly in that order with the clinical severity of disease and hence could be used as useful tools for assessment of severity of patients and the ranges corelating withseverity could be derived. Conclusion: Analyzing SpO2, radiological features, biochemical parameters individually and in combination with clinical severity and outcome in elderly patients helps in deriving biochemical and radiological parameters to predict severity which in turn helps in a holistic approach and right management at the right time preventing or reducing mortality due to Covid 19 in the elderly.

Objective: To explore the protective role of Glinus lotoides ethanolic extract in a depression model through modulating oxidant/antioxidant enzyme system and inflammatory status. Methods: Phytochemical constituents of Glinus lotoides ethanolic extract were evaluated qualitatively and quantitatively along with HPLC. Rats were divided into six groups. The normal control and the intoxicated groups received normal saline, and the standard group received imipramine, while the remaining groups received 100, 300, and 500 mg/kg Glinus lotoides ethanolic extract. All groups received treatments for 14 d. Lipopolysaccharides (LPS) were then administered i.p. (0.83 mg/kg) to all groups except the normal control group. After 24 h, anxiety and depression-like behaviors were evaluated by performing behavioral analysis (open field, tail suspension, forced swim, sucrose preference test), and determining total oxidant status, total antioxidant capacity, catalase, and biochemical parameters [malondialdehyde, glutathione, superoxide dismutase, tumor necrosis factor (TNF)-alpha and interleukin (IL)-6]. Results: Phytochemical studies confirmed the presence of phenols and flavonoids and HPLC analysis showed the presence of gallic acid, quercetin, chlorogenic, and caffeic acid. Total oxidant status was significantly decreased, while total antioxidant capacity was significantly increased in the Glinus lotoides ethanolic extract treated groups. Moreover, Glinus lotoides ethanolic extract diminished malondialdehyde, IL-6, and TNF-alpha levels, while increasing superoxide dismutase, catalase, and glutathione activities. Conclusions: Glinus lotoides ethanolic crude extract shows significant antidepressant activity by modulating oxidative and biochemical parameters that supports its folkloric use in traditional systems of medicine.

ABSTRACT

Tradicionalmente se han utilizado algunos índices antropométricos para el diagnóstico de exceso de peso en niños y adolescentes que han mostrado algunas desventajas por lo que se han postulado otros indicadores. En ese sentido, se plantea estimar el nivel de asociación entre indicadores antropométricos y la presencia de dislipidemia en adolescentes y adultos jóvenes. Se realizó una investigación observacional, descriptiva y de corte transversal en 123 adolescentes (68,2% mujeres, edad promedio 14,5 años) y 122 adultos jóvenes (70,5% mujeres, edad promedio 21 años) de la ciudad de Caracas. Se calcularon Índices de Masa Corporal (IMC), Índice Cintura­Talla (ICT), Índice de Masa Corporal Abdominal (IMCA) e Índice de Masa Tri-Ponderal (IMT). Se obtuvo una muestra de sangre por punción venosa, en ayuno de 12 a 14 horas, a partir de la cual se cuantificó Colesterol Total, Lipoproteína de alta densidad y Triglicéridos. Se calculó la concentración de Lipoproteína de baja densidad por la fórmula de Friedewald, así como el índice LDL-C/HDL-C y el índice LogTg/HDL. Para el análisis e interpretación de los datos se utilizó estadística descriptiva univariante y multivariante. Los resultados revelaron que los índices antropométricos IMCA e IMT no mostraron mejor desempeño en predecir dislipidemia que los indicadores IMC, Circunferencia de Cintura (CC) e ICT en adolescentes y adultos jóvenes. Los indicadores antropométricos de adiposidad abdominal, CC e ICT, tendieron a presentar mayores OR, ABC, sensibilidad y especificidad independientemente del grupo de estudio. En general, la capacidad de los indicadores antropométricos evaluados en predecir la presencia de dislipidemia en adultos jóvenes fue adecuada, situación que no se presentó en los adolescentes(AU)


Traditionally, some anthropometric indices have been used for the diagnosis of excess weight in children and adolescents, which have shown some disadvantages for which other indicators have been postulated. In this sense, it is proposed to estimate the level of association between anthropometric indicators and the presence of dyslipidemia in adolescents and young adults. An observational, descriptive cross-sectional investigation was carried out in 123 adolescents (68,2% women, media age 14,5 years) and 122 young adults (70,5% women, media age 21 years) from the city of Caracas. Body Mass Indices (BMI), Waist-Height Ratio (WHR), Abdominal Body Mass Index (BMAI) and Tri-Ponderal Mass Index (TMI) were calculated. A blood sample was obtained by venipuncture, fasting for 12 to 14 hours, from which Total Cholesterol, High Density Lipoprotein and Triglycerides were quantified. The low-density lipoprotein concentration was calculated by the FriedEwald formula, as well as the LDL-C / HDL-C index and the LogTg / HDL index. Univariate and multivariate descriptive statistics were used for the analysis and interpretation of the data. The results revealed that the BMI and TMI anthropometric indices did not show better performance in predicting dyslipidemia than the BMI, Waist Circumference (WC) and WHR indicators in adolescents and young adults. The anthropometric indicators of abdominal adiposity, WC and WHR, tended to present higher OR, AUC, sensitivity and specificity regardless of the study group. In general, the capacity of the anthropometric indicators evaluated to predict the presence of dyslipidemia in young adults was adequate, a situation that did not occur in adolescents(AU)

ABSTRACT

Physical exercise is effective in modulating circulating inflammatory markers of obesity. However, little is known about the effects of interventions with physical exercise programs accompanied by nutritional and/or psychological guidance, configuring themselves as multicomponent programs. Thus, the aim of this review is to systematically evaluate the evidence related to the effects of mul-ticomponent weight-loss interventions in modulating circulating inflammatory markers in children and adolescents. Therefore, the following databases will be searched to identify all relevant articles: PubMed, SciELO, Lilacs, Web of Science, EMBASE, Scopus, SPORT Discus. Randomized con-trolled trials and quasi-experimental studies of children and adolescents (6 to 18 years old) will be included. Eligible interventions will target weight-related behaviors (including diet, physical activity, behavior modification and/or combinations thereof ). Two independent reviewers will select studies using Rayyan QCRI software and extract the data to a standard form. The main outcomes of the review will be the circulating values of leptin, interleukin 6 (IL-6) and tumor necrosis factor-alpha (TNF- α) in a quantitative way. To assess the methodological quality (or risk of bias) of individual studies, Effective Public Health Practice Project assessment tool will be used. The meta-analysis will be performed using the Review Manager software


O exercício físico é eficaz na modulação dos marcadores inflamatórios circulantes da obesidade. Porém, pou-co se sabe sobre os efeitos de intervenções com programas de exercícios físicos acompanhados de orientações nutricionais e/ou psicológicas, configurando-se como programas multicomponentes. Assim, o objetivo des-te protocolo é propor uma revisão sistematicamente das evidências relacionadas aos efeitos de intervenções multicomponentes para perda de peso na modulação de marcadores inflamatórios circulantes em crianças e adolescentes. Para tanto, as seguintes bases de dados serão pesquisadas para identificar todos os artigos relevantes: PubMed, SciELO, Lilacs, Web of Science, EMBASE, Scopus, SPORT Discus. Serão incluídos ensaios clínicos randomizados e estudos quase experimentais com crianças e adolescentes (6 a 18 anos). As intervenções elegíveis terão como foco a melhora dos comportamentos relacionados ao peso (incluindo dieta, atividade física, modificação de comportamento e/ou combinações dos mesmos). Dois revisores independentes selecionarão estudos usando o software Rayyan QCRI e extrairão os dados em um formulário padrão. O principal resultado da revisão serão os valores circulantes de leptina, interleucina 6 (IL-6) e fator de necrose tumoral alfa (TNF-α) de forma quantitativa. Para avaliar a qualidade metodológica (ou risco de viés) de estudos individuais será utilizada a ferramenta de avaliação Effective Public Health Practice Project. A meta-análise será realizada no software Review Manager

ABSTRACT

Objective:To evaluate the clinical efficacy of Qingkailing injection in the treatment of children with sepsis and heat syndrome, and investigate its anti-inflammatory, anticoagulant and protective effects. Method:Eighty patients were randomly divided into control group and observation group with forty cases in each group according to the number table. Both groups received comprehensive treatment measures such as fluid resuscitation, anti-infection, anti-inflammatory, anticoagulation, vasoactive drugs, and protection of vital organ functions. While patients in observation group additionally took Qingkailing injection, 5-10 mL each time, intravenous drip after dilution, 1 time/day. Treatment course was five days in both groups. Before and after treatment, the scores of quick sequential organ failure assessment (qSOFA), pediatric critical illness score (PCIS) and acute physiology and chronic health evalution Ⅱ (APACHEⅡ) were graded; procalcitonin (PCT), serum amyloid A protein (SAA) before and after treatment, heparin-binding protein (HBP), tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>), high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), IL-10, N-terminal brain sodium Peptide precursor (NT-proBNP), high-sensitivity cardiac troponin T (hs-cTnT) level, cardiac troponin I (cTnI), creatine kinase isoenzyme (CK-MB), <italic>D</italic>-dimer (<italic>D-</italic>D ), fibrinogen (FIB) and antithrombin Ⅲ (AT-Ⅲ) levels were detected. Result:The APACHEⅡ and qSOFA scores in the observation group were lower than those in the control group (<italic>P</italic><0.05), while the PCIS score was higher than that in the control group (<italic>P</italic><0.05). The levels of PCT, SAA,HBP,TNF-<italic>α</italic>, hs-CRP and IL-6 in the observation group were lower than those in the control group (<italic>P</italic><0.01), while the IL-10 level was higher than that in the control group (<italic>P</italic><0.01). The levels of NT-proBNP, hs-cTnT, cTnI, CK-MB, <italic>D</italic>-D and FIB in the observation group were lower than those in the control group (<italic>P</italic><0.01), while the AT-Ⅲ activity was higher than that in the control group (<italic>P</italic><0.01). Conclusion:Qingkailing injection as the adjuvant therapy in children with sepsis and fever syndrome, can play the role of anti-inflammatory, anticoagulant, reducing infection and myocardial damage, thereby reducing the severity of the disease and improving the prognosis.

ABSTRACT Phagocytic functions by neutrophils/ monocytes and biochemical parameters were assessed in peripheral blood of patients with periodontitis, whether or not associated to type 2 diabetes, or patients with type 2 diabetes, or systemically healthy people. Fifty-eight participants were divided into four groups: Control - systemically and periodontally healthy patients (C, n=16), Periodontitis (P, n=14), Type 2 Diabetes (DM, n=11) and Periodontitis associated with type 2 diabetes (DMP, n=17). Blood samples were used to analyze phagocytic activity and the production of superoxide anion using optical microscopy. Significantly lower phagocytic activity of neutrophils was observed in non-opsonized samples (p = 0.008, Kruskal- Wallis) of the periodontitis group and in opsonized samples (p = 0.029, Kruskal-Wallis) of the periodontitis associated with type 2 diabetes group when these groups were compared to the healthy individuals when a 20:1 yeast: phagocyte stimulus was used. Periodontitis patients, whether associated (p = 0.0007, sensitized; Kruskal-Wallis, 20:1) or not with diabetes (p = 0.018 and 0.0007, in the proportions 5:1 and 20:1 yeast: monocyte respectively in sensitized samples; Kruskal-Wallis) also showed lower phagocytic function of monocytes compared to the control group. There was no significant difference in the production of superoxide anion among the evaluated groups. Severe clinical attachment loss was associated with lower levels of HDL in periodontitis patients and a higher percentage of A1C in diabetes with periodontitis patients (p<0.05; Pearson and Spearman correlations, respectively). Patients with both associated diseases had higher levels of triglycerides and CRP (p<0.001, Kruskal-Wallis) compared to patients with diabetes only. The results of the present study suggest that periodontitis negatively interferes with the innate immune response and may represent a major risk of systemic complications such as cardiovascular disease in diabetic patients or even in healthy individuals.


RESUMO As funções fagocíticas de neutrófilos/monócitos e parâmetros bioquímicos foram avaliados no sangue periférico de pacientes com periodontite com ou sem diabetes do tipo 2, ou em pacientes com diabetes tipo 2, ou em pessoas saudáveis sistemicamente. 58 participantes foram divididos em quatro grupos: Controle - pacientes sistemicamente e periodontalmente saudáveis (C, n = 16), Periodontite (P, n = 14), Diabetes Tipo 2 (DM, n = 11) e Periodontite associada a diabetes tipo 2 (DMP, n = 17). Amostras de sangue foram usadas para analisar a atividade fagocítica e a produção de ânion superóxido por microscopia óptica. Observou-se menor atividade fagocítica dos neutrófilos em amostras não opsonizadas (p = 0,008, Kruskal-Wallis) do grupo periodontite e em amostras opsonizadas (p = 0,029, Kruskal-Wallis) do grupo periodontite associada ao diabetes tipo 2 quando esses grupos foram comparados aos indivíduos saudáveis sob um estímulo de levedura:monócito de 20:1. Pacientes com periodontite associada (p = 0,0007, sensibilizados; Kruskal-Wallis, 20: 1) ou não com diabetes (p = 0,018 e 0,0007, nas proporções 5: 1 e 20: 1 de levedura: monócito, respectivamente, em amostras sensibilizadas; Kruskal- Wallis) também demonstraram menor função fagocítica dos monócitos em comparação com o grupo controle. Não houve diferença significativa na produção de ânion superóxido entre os grupos avaliados. A perda de inserção clínica grave foi associada a níveis mais baixos de HDL na periodontite e maior percentual de A1C nos pacientes com periodontite associada ao diabetes (p<0,05; correlações de Person e Spearman, respectivamente). Os resultados do presente estudo sugerem que a periodontite interfere negativamente na resposta imune inata e pode representar um risco maior para complicações sistêmicas, como a doença cardiovascular, em pacientes com diabetes ou mesmo em indivíduos saudáveis.

Fruit purees can be added to diet as alternative sources of bioactive compounds for the prevention and/or improvement of the complications of metabolic syndrome. In this work we evaluated the effect of the intake of low-fat diets enriched with fruit purees (guava-strawberry, guava-blackberry, guava-soursop, guava-passion fruit) on the body weight and biochemical markers in metabolic syndrome analogy (MSA)-induced rats. The rats (n=6 for each treatment) were induced with a high fat diet and were injected with streptozotocin, one dose every week for 4 consecutive weeks after fasting overnight, then healthy rats were fed with standard diet and MS rats were fed with standard diet plus each of the fruit puree, for 4 weeks. As novel findings, the diet enriched with fruit purees was associated with a reduction in body weight (~13-21 %) and a control in the metabolism of glucose by decreasing plasma glucose (~5963 %). Also, there was a reduction in the total cholesterol, triacylglycerols, low-density lipoproteins, and low enzymatic activities of alanine aminotransferase, alkaline phosphatase and γ-glutamyl transferase, useful metabolites in the control of inflammatory processes in the liver. A notable improvement in the liver morphology was observed indicating that the treatments had a hepatoprotective effect. The diet enriched with guava-blackberry puree caused the best results on most biochemical markers of MS rats. Therefore, diets enriched with fruit purees can be an alternative for MS individuals for the control and improvement of the complications caused by this syndrome.


Los purés de frutas se pueden agregar a la dieta como fuentes alternativas de compuestos bioactivos para la prevención y / o mejora de las complicaciones del síndrome metabólico. En este trabajo evaluamos el efecto de la ingesta de dietas bajas en grasas, enriquecidas con purés de frutas (guayaba-fresa, guayaba-mora, guayaba-guanábana, guayaba-maracuyá) sobre el peso corporal y los marcadores bioquímicos en el síndrome metabólico (SM) inducido en ratas. Las ratas (n = 6 para cada tratamiento) fueron inducidas con una dieta alta en grasas y se les inyectó estreptozotocina, una dosis cada semana durante 4 semanas consecutivas después de ayunar durante la noche. Luego, las ratas sanas fueron alimentadas con una dieta estándar; y las ratas con SM fueron alimentadas con dieta estándar más cada uno de los purés de frutas, durante 4 semanas. Como hallazgos novedosos, la dieta enriquecida con purés de frutas se asoció con una reducción en el peso corporal (~ 13-21 %) y un control en el metabolismo de la glucosa al disminuir la glucosa en plasma (~ 59-63 %). Además, hubo una reducción en el colesterol total, triacilgliceroles, lipoproteínas de baja densidad, y bajas actividades enzimáticas de alanina aminotransferasa, fosfatasa alcalina y gama-glutamil transferasa, metabolitos útiles en el control de los procesos inflamatorios en el hígado. Se observó una mejora notable en la morfología del hígado, lo que indica que los tratamientos tuvieron un efecto hepatoprotector. La dieta enriquecida con puré de guayaba y mora causó los mejores resultados en la mayoría de los marcadores bioquímicos de las ratas con SM. Por lo tanto, las dietas enriquecidas con purés de frutas pueden ser una alternativa para las personas con SM, para el control y la mejora de las complicaciones causadas por este síndrome.

ABSTRACT

The objective of this study was to evaluate the enzymatic activity of plasma cholinesterase in Chelonia mydas marine turtles belonging to two populations, according to their capture sites, under the absence and probable influence of anthropic effects. A total of 74 animals were used and later divided into two groups, based on the capture site. Blood samples were collected from all captured animals, which were then released into the sea at the site of capture. A descriptive statistical analysis of the plasma cholinesterase activity values and an analysis comparing these values based on the capture site were performed. Samples of heparinized plasma from animals captured at the two different sites were analyzed. Plasma cholinesterase activity ranged from 121 to 248U/L, with a mean and standard deviation of 186.1±30.68U/L. When comparing plasma cholinesterase activity values in individuals based on the capture site, a significant difference was observed. Establishing reference values for different sea turtle populations is necessary to interpret future sampling results and to allow sea turtles to be used as sentinels of ecosystem health. Future studies are needed to evaluate other populations and the activity of plasma cholinesterase in juvenile marine turtles, in relation to environmental contamination.(AU)


O objetivo desse estudo foi avaliar a atividade enzimática da colinesterase plasmática em tartarugas marinhas da espécie Chelonia mydas em duas populações de acordo com o local de captura, sob ausência e provável influência de efeito antrópico. Foi utilizado um total de 74 animais e posteriormente divididos em dois grupos de acordo com o local de captura. Foram coletadas amostras de sangue de todos os animais capturados e em seguida liberados ao mar no mesmo local. Foi realizada uma análise estatística descritiva dos valores da atividade plasmática de colinesterase do total de animais e análise comparando os valores de acordo com o local de captura. Foram analisadas amostras de plasma heparinizado de animais capturados em dois locais distintos. Os valores da atividade plasmática de colinesterase variaram de 121 a 248U/L, com média e desvio padrão de 186.1±30.7U/L. Quando comparados os valores de atividade plasmática da colinesterase nos indivíduos de acordo com o local de captura, foi observada diferença significativa. O estabelecimento de valores de referência para diferentes populações de tartarugas marinhas são necessários para interpretar os futuros resultados amostrais e permitir que as tartarugas marinhas sejam usadas como sentinelas da saúde do ecossistema. Estudos futuros são necessários para avaliar outras populações e a atividade da colinesterase plasmática de tartarugas marinhas juvenis em relação à contaminação ambiental.(AU)

ABSTRACT Objective To measure type 1 serum amino-terminal propeptide procollagen (P1NP) and type 1 cross-linked C-terminal telopeptide collagen (CTX) before parathyroidectomy (PTX) in PHPT patients, correlating these measurements with bone mineral density (BMD) changes. Subjects and methods 31 primary hyperparathyroidism (HPTP) were followed from diagnosis up to 12-18 months after surgery. Serum levels of calcium, parathyroid hormone (PTH) vitamin D, CTX, P1NP, and BMD were measured before and 1 year after surgery. Results One year after PTX, the mean BMD increased by 8.6%, 5.5%, 5.5%, and 2.2% in the lumbar spine, femoral neck (FN), total hip (TH), and distal third of the nondominant radius (R33%), respectively. There was a significant correlation between BMD change 1 year after the PTX and CTX (L1-L4: r = 0.614, p < 0.0003; FN: r = 0.497, p < 0.0051; TH: r = 0.595, p < 0.0005; R33%: r = 0.364, p < 0.043) and P1NP (L1-L4: r = 0,687, p < 0,0001; FN: r = 0,533, p < 0,0024; TH: r = 0,642, p < 0,0001; R33%: r = 0,467, p < 0,0079) preoperative levels. The increase in 25(OH)D levels has no correlation with BMD increase (r = -0.135; p = 0.4816). On linear regression, a minimum preoperative CTX value of 0.331 ng/mL or P1NP of 37.9 ng/mL was associated with a minimum 4% increase in L1-L4 BMD. In TH, minimum preoperative values of 0.684 ng/mL for CTX and 76.0 ng/mL for P1NP were associated with a ≥ 4% increase in BMD. Conclusion PHPT patients presented a significant correlation between preoperative levels of turnover markers and BMD improvement 1 year after PTX.

ABSTRACT

La terapia de oxigenación hiperbárica (TOHB) es usada en el tratamiento de diferentes patologías y es prioritario conocer su injerencia en la bioquímica del paciente y establecer el perfil de seguridad. Con el objetivo de establecer la seguridad y conocer la bioquímica de seguimiento de pacientes que realizan el tratamiento, se realizó un estudio retrospectivo de individuos sanos evaluando el efecto de la TOHB (100% O2 a 1,4 atm) sobre algunos parámetros bioquímicos. Se estudió el estado basal, y a las 10 y 20 sesiones, de los valores de los indicadores de coagulación, hematológicos, función hepática y reactantes de fase aguda. En los 20 individuos sanos no hubo alteraciones en el examen clínico general debido a la exposición a la TOHB. La media (con DE) de los valores bioquímicos a las sesiones 0, 10 y 20 estuvieron siempre dentro del rango de valores de referencia. Estos resultados sugieren que el tratamiento en estas condiciones no produce cambios estadísticamente significativos en los parámetros estudiados.


Hyperbaric oxygenation therapy (HBOT) is used in the treatment of different pathologies and it is a priority to establish its safety profile. A retrospective study in healthy individuals was performed, evaluating the effect of HBOT at 1.4 ATA on biochemical indicators. The general and basal status and after 10 and 20 sessions of biochemical parameters in blood sample, on the biochemical markers of coagulation, hematologic values, hepatic function status, and acute reaction were analysed as well. For the 20 healthy subjects, there were no alterations in the general clinical examination due to exposure to HBOT. The mean values (with SD) of the biochemical parameters at 0, 10 and 20 sessions were mostly within reference values range. The results suggest that the treatment does not produce statistically significant changes in the biochemical parameters studied in these conditions.


A terapia de oxigenação hiperbárica (TOHB) é utilizada no tratamento de diferentes patologias e é prioritário conhecer sua ingerência na bioquímica do paciente e estabelecer o perfil de segurança. A fim de estabelecer segurança e conhecer a bioquímica de monitoramento de pacientes que realizam o tratamento, foi realizado um estudo retrospectivo de indivíduos saudáveis avaliando o efeito da OHB (100% O2 a 1,4 atm) em alguns parâmetros bioquímicos. O estado basal foi estudado e em 10 e 20 sessões os valores de coagulação, hematológicos, função hepática e reagentes de fase aguda foram estudados. Nos 20 indivíduos saudáveis, não houve alterações no exame clínico geral devido à exposição à TOHB. A média (com o DS) dos valores bioquímicos nas sessões 0, 10 e 20 estavam sempre dentro da faixa de valores de referência. Estes resultados sugerem que o tratamento sob estas condições não produz mudanças estatisticamente significativas ao nível dos parâmetros estudados.

Background: The elevation of troponin-T (Trop-t) and creatinine kinase myocardial isoform (Ckmb) and elevation of these markers lead to major adverse cardiac events (MACEs). The association between above-mentioned parameters along with electrocardiogram (ECG) changes can be suitable diagnostic tool for myocardial injury following percutaneous coronary intervention (PCI). The present study was attempted to know the association of the changes in surface ECG and cardiac biomarkers and MACEs following PCI with the outcome in follow up among patients in eastern India. Methods: In present study, 100 patients were randomly selected for coronary angioplasty between April2012 to March2013. All the cases were referred to catheterization laboratory for elective PCI for single vessel or multivessel in native coronary artery. The biomarkers such as trop-t and Ckmb and MACEs along with changes in ECG were estimated as per standard protocol. Results: ECG changes were statistically significantly correlated with post procedure Trop-t elevation (p<0.001), Ckmb elevation (p<0.001) and MACEs (P<0.01 and p<0.001) in 6 months followed up. Conclusion: In conclusion, the present study showed significant relationship between MACEs during followed up and changed in surface ECG along with Trop-t and Ckmb elevation in the post-PCI period of six-month followed up in stable ischaemic heart disease.

Background: Hypertension is the most common illness of circulatory system and is a worldwide health challenge affecting both developed and developing nations. The development of hypertension is a multi-factorial process and the risk factors for its development are many such as genetic factors, obesity, age & sex factors, salt sensitivity, dyslipidemia, hyperinsulinemia & insulin resistance, environmental factors, socio economic imbalances, free radicals/reactive Oxygen species, oxidative stress, blood groups etc. The role of blood groups in the etiology of essential hypertension has long been suspected. Methods: The study design was a cohort study consisting of 15 hypertensive patients of the A, B, AB and O each. Anthropometric parameters such as Blood pressure (SBP/DBP) and Body Mass Index (BMI) and Biochemical parameters such as Blood grouping, Blood Glucose levels, Haemoglobin levels, Lipid profile, which included Triglycerides, Total cholesterol, HDL cholesterol and LDL and VLDL cholesterol, were measured using standardized methods. The various parameters were compared between hypertensives of the various blood groups and statistical analysis was done using SPSS. Mean and SD of each group was calculated, and Unpaired student’s t-test was applied (P < 0.05 considered as significant). Results: When BMI was compared among hypertensive groups we observed significant increased level of BMI in group A as compared to group O hypertensives. (10.36%; P<0.05). While comparing SBP between hypertensive groups we observed significant raised level of SBP in group A as compared to group B (9.38%; P<0.01) and O (10.45%; P<0.01). Similarly, DBP was found to be increased significantly in group A as compared to group B ((5.34%; P<0.05) and O hypertensive patients (5.06%; P<0.001). LDL-C and HDL-C were non-significantly altered in these groups however value of VLDL-C was found increased in group A as compared to group O hypertensives (20.38%; P< 0.05). Conclusion: The study concluded that there is a significantly higher SBP and DBP in the hypertensives of the A blood group as compared to the hypertensives of the B and O blood groups. It also found increased levels of serum triglycerides, total cholesterol, LDL-C, VLDL-C along with decreased levels of HDL-C was in hypertensive patients predominantly in group A and AB hypertensive. The hypertensive patients of A blood group have shown a statistically significant increase in BMI as compared to hypertensives of the other blood groups.

Introdução: o Infarto Agudo do Miocárdio (IAM) é a mais importante e frequente cardiopatia isquêmica ocasionada por necrose do músculo cardíaco (miocárdio) em consequência de isquemia prolongada, sendo mais frequente em adultos e idosos. O diagnóstico é baseado no quadro clínico, alterações eletrocardiográficas e resultados de exames de sangue com elevação dos marcadores bioquímicos de necrose miocárdica: Troponina T, CKMB (creatina-quinase-fraçãoMB) e CK (creatina-quinase-total) que são liberadas pelas células assim que o suprimento de sangue fica bloqueado, sendo a Troponina o principal marcador por estar presente na estrutura do músculo cardíaco. A bioquímica clínica desempenha papel fundamental no diagnóstico laboratorial do IAM, pois as dosagens devem ser analisadas rapidamente para melhor prognóstico, eficácia do tratamento e sobrevida do paciente. Embasados na alta prevalência das doenças cardiovasculares, no envelhecimento populacional e na importância do diagnóstico laboratorial. O objetivo desta revisão bibliográfica foi descrever a importância dos principais marcadores bioquímicos quanto à sensibilidade e especificidade com ênfase para a Troponina T. Trata-se de um estudo transversal descritivo por revisão da literatura de artigos científicos completos, capítulos de livros, dissertações e teses nos idiomas português e inglês, nas bases de dados SCIELO, LILACS, BIREME, BIBLIOTECA VIRTUAL DE SAÚDE. Após a revisão, evidencia-se a importância da emergência médica com o diagnóstico laboratorial preciso e confiável da Troponina T que é liberada em 3 horas após o IAM e que pode ser dosada por imunocromatografia ou quantitativamente, e deve estar inserida na rotina dos laboratórios privados e hospitalares para melhor qualidade de vida do paciente.


Introduction: acute Myocardial Infarction (AMI) is the most important and frequent ischemic heart disease caused by cardiac (myocardial) muscle necrosis as a consequence of prolonged ischemia, being more frequent in adults and the elderly. The diagnosis is based on the clinical picture, electrocardiographic changes and results of blood tests with elevation of biochemical markers of myocardial necrosis: Troponin T, CKMB (creatine kinase-MB fraction) and CK (creatine kinase-total) that are released by the cells as soon as the blood supply is blocked, Troponin being the main marker because it is present in the structure of the heart muscle. Clinical biochemistry plays a fundamental role in the laboratory diagnosis of AMI, since the dosages should be analyzed quickly for better prognosis, treatment efficacy and patient survival. Based on the high prevalence of cardiovascular diseases, population aging and the importance of laboratory diagnosis, the objective of this literature review was to describe the importance of the main biochemical markers for sensitivity and specificity with emphasis on Troponin T. This is a cross-sectional study descriptive literature review of complete scientific articles, book chapters, dissertations and theses in the Portuguese and English languages, in the databases SCIELO, LILACS, BIREME, VIRTUAL HEALTH LIBRARY. After the review, the importance of the medical emergency with the accurate and reliable laboratory diagnosis of Troponin T, which is released within 3 hours after AMI and can be quantified by immunochromatography or quantitatively, should be included in the routine of the private and the quality of life of the patient.
